CSB Safety Videos Receive Awards at Annual Television, Internet, and Video Association of DC (TIVA DC) Banquet

The U.S. Chemical Safety Board (CSB) received awards for two safety videos at the annual Television, Internet, and Video Association of DC (TIVA DC) dinner on Saturday, November 14.

CSB to hold Public Meeting on November 23, 2015, in Washington, DC

The Board will discuss investigations and operational activities through the end of the calendar year and will also provide an update on CSB audits conducted by the EPA Inspector General. The agenda will also include a discussion of future public meetings to explore worker fatigue and process safety management issues, as well as a staff presentation and Board vote on a recommendation to the BP Global Executive Board of Directors to implement an incident reporting program.

On behalf of everyone at the U.S. Chemical Safety Board, I would like to express our sympathies to the students and teacher who were injured at W.T. Woodson High School in Fairfax County, Virginia, last Friday, October 30, 2015. The CSB is not investigating this accident, but is gathering facts about the incident. The agency plans to increase its advocacy around this issue to promote our key lessons going forward.

CSB Approves Final Investigation Report and Video from 2009 Explosion and Fire at Caribbean Petroleum Terminal Facility in Puerto Rico; Report Finds Inadequate Management of Gasoline Storage Tank Overfill Hazard

The US Chemical Safety Board (CSB) today voted on the final investigation report into the 2009 massive explosion at the Caribbean Petroleum, or CAPECO, terminal facility near San Juan, Puerto Rico; the report includes recommendations for addressing regulatory gaps in safety oversight of petroleum storage facilities by the Occupational Safety and Health Administration (OSHA) and Environmental Protection Agency (EPA).

CSB to hold Public Meeting on October 21, 2015, in Washington, DC

The U.S. Chemical Safety Board (CSB) will convene a public meeting on October 21, 2015, starting at 1 p.m. EDT in Washington, DC at the Palomar Hotel, 2121 P St, NW, in the Phillips Ballroom. The Board will discuss the final report and recommendations from the Caribbean Petroleum incident, and may then vote on the Caribbean Petroleum report. The Board will also discuss the status of several current CSB investigations and the agency action plan for FY 15.

The interim recommendations stem from the ongoing investigation by the CSB of the November 15, 2014, toxic chemical leak that killed four workers at the DuPont insecticide plant in La Porte, Texas.
